Section 1, Basic Information

To insure we have the correct spelling of you name, your correct address and phone numbers.  

Section 2, Credit Terms

If you wish an Open Account (or Charge Account) or a Charge Card Account complete this section.  A Charge Card Account reverts to either a COD or a Charge Account when the card expires or is discontinued, based on the information you provide.

Section 3, Online Account

Everyone can benefit from an Online Account, even if you do not intend to make online payments.  All we need is an email address.  In addition to invoices and schedules, you’ll see tech licenses, labels and various other reports.

Section 4, Your Signature

By signing, you agree to pay within your terms and to pay late fees should your account become delinquent.  You are also stating that the information you are giving is true and accurate.
